Pneumonia is a lung problem, infection of the lungs where fluid builds up. Ultra-microscopic = very small, very fine. Silica = a mineral. Volcano = lava shoots out. So hrmm a pneumonia sort of condition caused by breathing dust or smoke after a volcano eruption? Sounds like it would be nasty.
EDIT: more specifically breathing in silica dust or some shit, that is present in or around volcanoes, probably the dust brought up from eruptions and not the smoke, that then causes chronic irritation in the lungs and makes it hard to breathe or something.
